Request Reports

You may visit our police department at 151 James Street Gray, Georgia 31032 or by mailing us an accident or incident report request with a self addressed stamped envelope (please include the name of the person involved, date of the accident/incident, and any other information that may assist us in locating your report).

Accident/Incident reports can be requested three business days after the accident/incident date by telephone, fax, or in person during regular office hours.
